3. Inclusion of observations

Describe what you saw: Include a detailed description of your observations during and after

the incident. For example, describe the appearance, behaviour, or actions of the individuals

involved, the condition of the environment, or any physical evidence present.

Document any injuries or damages: If there were injuries or property damages, provide

a clear description, including the extent of injuries or the nature of the damage. Be specific

about the location, nature, and severity of any harm.

   4. Actions Taken

Document immediate actions: Record any actions you or others took during the incident,

such as calling for medical assistance, attempting to de-escalate a situation, or securing the

area. Specify who took these actions and what the outcomes were.

Follow-up actions: Mention any follow-up actions taken after the incident, such as

reporting to authorities, conducting an investigation, or notifying relevant personnel.

Include any steps taken to prevent recurrence or address the consequences of the incident.

   5. Involvement of Other Parties

Identify witnesses: List the names and contact information of any witnesses to the

incident. Include a summary of their statements if they provided any.

Involvement of authorities or emergency services: If law enforcement, medical

personnel, or other authorities were involved, include their names, badge numbers, and

actions they took. If reports were filed by these entities, mention them.
